Dear parent,
This message is circulated once again as many parents still have doubts regarding the medication of students during school hours.
We request parents to give any medication at home as much as possible. As antibiotics are typically taken on 8 or 12 hour cycles, it has to be administered before the child comes to school.
If your child needs to take any medication during school hours, the medicines should be brought to the school nurse in its original container by the parent/ guardian along with
(A) a copy of the prescription from physician and
(B) the signed medication consent form by the parent/ guardian.

You can download a copy of the parental consent form to administer prescribed medication here.
In the absence of the doctor’s prescription or the signed medication consent form, medicines will not be administered due to HAAD and insurance regulations.
